$5 Exam Rebate — Align your Johnson & Johnson Vision and CooperVision accounts with PECAA Max (direct or distributor purchases).
$5 Enhancement — Align at least two frame vendors with PECAA Max — choose from Marchon, Altair, Safilo, Luxottica.
$5 Marchon | Altair Enhancement — Receive an Additional $5 when both Marchon and Altair are aligned with PECAA Max.
Rebate is paid for every VSP commercial patient who receives eye exam services with a claim paid through December 31, 2027.
To qualify for the Rebate Enhancement, you must also qualify for the Exam Rebate.
The maximum amount payable through the Vision Exam Rebate and Rebate Enhancements in 2026 and 2027, respectively, is as follows:

2026 — maximum payable is based on PECAA Max membership effective date:

Members joining in 2027 will be eligible to receive a maximum of $30,000 next year, regardless of lab alignment.
There is no requirement to participate in VSP Premier Edge™ or any spend requirement with VSP supply chain products.

Terms & Conditions

Must be a PECAA Max Member in good standing to qualify. The Vision Exam Rebate Offer is valid for eligible exams paid from July 1, 2023, through December 31, 2027. The Rebate Enhancement Offer is valid for eligible exams paid from March 1, 2024, through December 31, 2027. Marchon | Altair Enhancement Offer is valid for eligible eye exams paid from July 1, 2025, through December 31, 2027. Lab Partner Bonus Cap is valid for eligible eye exams paid from January 1, 2027, through December 31, 2027, for Members aligned with ABB Labs, HOYA, or ZEISS (“Lab Alignment”). To earn the Lab Partner Bonus Cap, Member must be a PECAA Max Member with Lab Alignment in place no later than 4:00 PM Pacific Time on December 31, 2026. Only claims for exams performed on VSP patients who are covered by VSP commercial insurance plans are eligible for this rebate offer. Claims paid by state or federal healthcare funds are not eligible. Member must meet the above-stated eligibility criteria for the Vision Exam Rebate, the Enhancement(s), and the Lab Partner Bonus Cap by aligning the qualifying vendors with their PECAA Max Membership. To qualify for the $5 Rebate Enhancement associated with aligning the designated frame vendors, Member must also qualify for the $5 Rebate associated with aligning the designated contact lens vendors. To qualify for the $5 Marchon | Altair Enhancement associated with aligning Marchon | Altair, Member must also qualify for the $5 Rebate associated with aligning the designated contact lens vendors. To qualify for the Lab Partner Bonus Cap associated with aligning with a PECAA Max strategic lab partner, Member must also qualify for the $5 Rebate associated with aligning contact lens vendors. Lab Alignment must be in place no later than 4:00 PM Pacific Time on December 31, 2026 to qualify. Member must be enrolled with PECAA Max and have completed the process to align the qualifying vendor accounts with their PECAA Max Membership by 4:00 PM Pacific Time on the 25th of the month to be eligible to receive the Exam Rebate and Enhancements for VSP commercial exams paid from the 1st day of the same month. Should the 25th of the month fall on a non-business day for PECAA, the deadline to complete the process to align qualifying vendor accounts shall be 4:00 PM Pacific Time on the last business day of the month prior to the 25th. For 2026: For practices enrolled in PECAA Max prior to January 1, 2026: The aggregate maximum amount payable under the Vision Exam Rebate and the Enhancement(s) is $60,000 for 2026. Vision Exam Rebate offer is capped at $30,000 per Member account for claims paid during 2026. The Enhancements offer is capped at $30,000 per Member account for claims paid during 2026. For practices enrolled in PECAA Max January 1, 2026, or later: The aggregate maximum amount payable under the Vision Exam Rebate and the Vision Exam Rebate Enhancement(s) per Member account is $30,000 for 2026. Vision Exam Rebate offer is capped at $15,000 per Member account for claims paid during 2026. The Enhancements offer is capped at $15,000 per Member account for claims paid during 2026. For 2027: The aggregate maximum amount payable under the Vision Exam Rebate and the Enhancements is $30,000 for 2027. The Vision Exam Rebate offer is capped at $15,000 per PECAA Max Member account for claims paid during 2027. The Enhancements offer is capped at $15,000 per PECAA Max Member account for claims paid during 2027. Lab Partner Bonus Cap: PECAA Max Members who joined on or before December 31, 2026, may increase the aggregate maximum to $60,000 for 2027 by aligning with one of PECAA Max’s strategic lab partners: ABB Labs, HOYA, or ZEISS. For Members who qualify for the Lab Partner Bonus Cap, the Vision Exam Rebate offer is capped at $30,000 per Member account for claims paid during 2027 and the Enhancements offer is capped at $30,000 per Member account for claims paid during 2027. To receive payment, Member must remain aligned with the applicable qualifying vendors at the time the Vision Exam Rebate and Enhancements, including the Lab Partner Bonus Cap, are paid out. Rebates paid under the Exam Rebate, Enhancements, and Lab Partner Bonus Cap (“the Rebates”) are subject to applicable taxes; a 1099 will be issued to all rebate recipients. Members are responsible for any required tax reporting and/or payment of any applicable taxes associated with the Rebates.
